Hybrid and Total Endovascular Approaches to Tandem Carotid Artery Lesions Have Similar Short- and Long-Term Outcomes

A retrospective review of the Vascular Quality Initiative database was performed from 2005 to 2020. Of 1433 patients with atherosclerotic tandem lesions in internal carotid artery (ICA) and proximal common (CCA) or innominate artery, 795 (55%) had undergone antegrade transfemoral stenting ICA lesion 638 (45%) hybrid interventions consisting endarterectomy (CEA) retrograde endovascular stenting. Perioperative stroke/death (∼3.8%) 1-year freedom stroke/reintervention/death (∼93%) were same for both strategies. Although simultaneous repair portends worse outcomes compared CEA (CAS) alone, no difference found short- long-term stroke death rates a totally approach.
